- For clients eligible for federal or provincial benefit programs, they can submit Membership Assistance Form ( https://www.medicalert.ca/medicalert/media/forms/MedicAlert-Membership-Assistance-Form.pdf ) for receiving the services for no cost, which will take 3 weeks for approval.
